Elliot and Carla sets out on a mission to get the board to let Dr. Kelso stay on as Chief Of Medicine while Dr. Kelso himself thinks about his time at Sacred Heart.

J.D. starts to have doubts about his masculinity after Dr. Cox mocks him about it. Turk on the other hand tries to overcompensate his masculinity with the break room remote. The Janitor starts his own hospital newspaper, called “The Janitorial”.

It is the anniversary for Carla and Turk’s first date and both work on getting each other the perfect gift. When Turk decides to learn Spanish, he soon realizes that being bilingual can only work to his advantage. Meanwhile, J.D. is torn about whether he should allow one of his patients, a burn victim, to attend his graduation and turns to Elliot for advice. Elsewhere, Dr. Cox decides to steal all of Dr. Kelso’s food.

Though Elliot boasts that through private practice she gets to get to know her patients, nobody seems to care. Carla is amazed at how the Janitor was able to score such an attractive girlfriend. The doctors are improving their services to get a higher ranking on a website and although J.D. appears nonchalant initially, he is elated when he is ranked #1.

J.D. and Turk take a trip down memory lane to their first prank together in the midst of planning their next one. Dr. Cox has an 11-year-old patient, Josh, with leukemia. Dr. Cox has to deal with the child’s parents who want to keep the condition a secret from Josh. On Dr. Kelso’s birthday, Elliot throws him a party.
